Primary care urgent referral form
The primary care urgent referral form should be completed by primary care colleagues across Oldham, Rochdale and Bury to refer patients from their practice into the specialist sexual health service.
Urgent referrals can be made for the patients below:
- Patients who have tested positive for Gonorrhoea/Syphilis or HIV
- Patients who require an IUD (copper coil) for emergency contraception
This form should be completed by a registered professional.
We aim to contact your patients within 48 hours of a referral (or the next working day if this is a bank holiday or weekend) appropriate to the reason for referral.
Targeted 1-1 education support referral
We provide bespoke, targeted 1-1 sessions between the young person and one of our Education and Wellbeing Specialist Workers. Usually this will be for an average of 6 sessions (with each session lasting between 30 minutes and 1 hour), although shorter interventions can be developed if necessary.
Sessions can be arranged at a number of venues such as:
- In schools or colleges
- Bury or Rochdale sexual health services
- A drop in clinic
- A mutually agreed venue
We aim to provide quick, easy, and confidential access to our service. The best way to make a referral is to review the referral guidance document and complete the referral form. Once completed, please send via email to firstname.lastname@example.org. If you need to discuss a referral or you have a query, please contact 01706 202 444.
